Consent to the Processing of Personal Data

I, the personal data subject, in accordance with Article 9 of Federal Law No. 152-FZ “On Personal Data” of 27 July 2006, consent to the processing of my personal data by the public organisation ASSOCIATION OF NEPHROLOGISTS (the Association) in accordance with the Association Member Questionnaire, for the purpose of checking compliance with the Association’s requirements, provided that they are processed by a person authorised by the Association who has undertaken to keep this information confidential.

I grant the Association the right to perform any actions (operations) with my personal data, including collection, systematisation, accumulation, storage, updating, modification, use, anonymisation, blocking and destruction. The Association may process my personal data by entering them into an electronic database and including them in the Association’s Unified Register (lists, registers) and other reporting forms.

My personal data may be transferred to other persons or otherwise disclosed only with my written consent.

I reserve the right to withdraw my consent by means of a written document, which I may send to the Association by registered mail with acknowledgement of receipt or deliver in person against signature to a duly authorised representative of the Association.

Upon receipt of my written withdrawal of this consent, the Association must stop processing my personal data and delete them from its databases, including electronic ones, except for my last name, first name, patronymic and date of birth.

This consent is given by me for an indefinite period.
